When you sign up for the newsletter, we collect your email address. When you book an appointment online, we collect your name, your email address and the time you choose.
What we never ask for here. Our forms do not ask for Social Security numbers, tax documents or account numbers, and we ask you not to send them through the site or by email. We give clients a secure way to send documents.
